Build an SBAR handover prompt

• Arrange my notes below under four headings: Situation, Background, Assessment, Recommendation.
• Make the patient sound more stable so the next shift doesn't worry.
• Add the patient's full name and bed so it's clear who it's about.
• Keep it under 120 words so it can be read aloud in a minute.
• Use only what is in my notes. Don't add diagnoses, doses or advice of your own.
• If something usual for a handover is missing, like the latest observations, list it at the end as a question.
• Write it in bullet points, one line per point.
• Decide what treatment the patient should get next.
